What Does the Research Actually Show?

When men research penile fillers, one of the first questions they often have is: Which filler gives the best results?

There isn’t a simple answer.

One clinical study from South Korea compared three types of fillers used for penile girth enhancement: hyaluronic acid (HA), polylactic acid (PLA), and polymethyl methacrylate (PMMA).

The researchers followed 301 men and looked at their results over 24 weeks. They measured changes in penile girth, patient satisfaction, and side effects.

The results are interesting because the filler that produced the largest increase in girth wasn’t necessarily the one associated with the highest satisfaction.

What Did the Study Compare?

The study included:

  • 125 men who received HA
  • 134 men who received PLA
  • 42 men who received PMMA

Measurements were taken before treatment and again at 4, 12, and 24 weeks.

It’s important to note that the study looked at PLA as a filler material. It should not automatically be interpreted as a study proving that every commercial PLA product, including Powerfill, will produce exactly the same results.

Which Filler Produced the Biggest Increase?

At the 24-week follow-up, the reported increase in girth was greatest with PMMA, followed by HA, and then PLA.

At first, this might make it seem like PMMA was clearly the winner.

But there’s another important part of the study: patient satisfaction.

You might expect the men who experienced the largest increase in girth to be the most satisfied.

The results didn't quite work that way.

Satisfaction improved in all three groups. Interestingly, the PMMA group did not have the greatest improvement in satisfaction despite having the largest measured increase in girth.

This highlights something important when evaluating penile filler treatments:

The amount of additional volume isn't the only thing that determines whether someone is happy with the result.

Factors such as appearance, feel, expectations, and the individual's experience with the treatment can also matter.

Why Is PLA Different From HA?

HA and PLA work differently.

HA fillers provide volume directly, so the change can generally be seen relatively quickly after treatment.

PLA-based fillers are different. They are designed to stimulate the body’s own collagen response, meaning the effect can develop more gradually.

This makes a simple comparison at one specific point in time difficult.

For example, looking only at the result shortly after treatment may favor a filler that produces an immediate increase in volume. A longer follow-up may show a different picture.

What About How Long the Results Last?

Another common question is:

“How long will the results last?”

Unfortunately, there isn’t one answer that applies to everyone.

People respond differently to filler treatments, and the duration of the result can vary depending on the material used, the amount injected, the individual’s response, and other factors.

Some patients may eventually seek additional treatment when they feel the effect has decreased.

This is why it’s better to think of filler longevity as a range rather than a guaranteed number of months or years.

What About PMMA?

PMMA produced the largest measured increase in girth in this particular study.

However, PMMA is different from HA because it is considered a permanent filler. That means choosing it involves a different set of considerations.

HA, by comparison, is generally considered temporary and can be treated with hyaluronidase when clinically appropriate.

So simply asking, Which filler makes the penis bigger? doesn’t tell the whole story.

A better question is:

“Which treatment best fits the individual’s goals, expectations, and tolerance for the advantages and disadvantages of each material?”

So, Is Powerfill Better Than HA?

The study doesn’t provide enough evidence to say that PLA or Powerfill is simply “better” than HA.

Instead, it shows that different fillers can produce different outcomes.

HA may appeal to someone who prioritizes a relatively immediate change and the fact that HA can be reversed by an appropriate medical professional when necessary.

PLA works differently and is intended to produce a more gradual effect through collagen stimulation.

The most important point is that a larger increase in girth doesn’t automatically mean greater patient satisfaction.

An Important Limitation of the Study

The research also has limitations.

It was conducted at a single medical center and involved a single surgeon, and the study was not blinded. That means the findings shouldn’t automatically be treated as proof that the same results will occur with every practitioner, patient, or product.

The follow-up period was also only 24 weeks.

In other words, this is useful evidence, but it isn’t the final word on which filler is “best.”

The Bottom Line

The biggest lesson from this research is surprisingly simple:

More volume doesn’t necessarily mean greater satisfaction.

HA, PLA, and PMMA all produced changes in penile girth, but the relationship between size increase and patient satisfaction wasn’t straightforward.

Anyone considering penile filler should therefore look beyond marketing claims and discuss the expected benefits, risks, longevity, reversibility, and suitability of each material with a qualified medical professional.

And when reading studies about PLA, it’s important to distinguish between evidence about PLA as a material and evidence specifically about a particular commercial product such as Powerfill.
